Corrupted somthing or another affecting confirmation email

I had a new user forward a copy of the email he received after subscribing. 

It appears the program is not picking up the subscriber information in his confirmation, but instead is sending the hooks to the data.

BTW -  I do not use the extra subscriber fields at all and don't have them on the sub form.

Using Dada 2.11.16
