It's closed - turned out to be a one-line fix In:
dada/templates/filter_using_black_list
tmpl
Find,
<a href="<!-- tmpl_var S_PROGRAM_URL -->?flavor=blacklist_filter_form" class="small button">
and change it to,
<a href="<!-- tmpl_var S_PROGRAM_URL -->?flavor=filter_using_black_list" class="small button">
The rest of the changes in the attached commit are to make things look/work nicer/better,
