modal box for subscription form

I can’t get the modal box to work and I can’t figure out why. It works in preview mode but not on the live subscription form. I have this in the head:

<!-- Dada Modal Scripts -->
<script src="/dada_mail_support_files/static/javascripts/vendor/jquery-2.1.4.min.js"></script>
<link rel="stylesheet" href="/dada_mail_support_files/static/css/vendor/colorbox/colorbox.css">
<script src="/dada_mail_support_files/static/javascripts/vendor/jquery/colorbox/jquery.colorbox-min.js"></script>
<script src="/dada_mail_support_files/static/javascripts/jquery.dadamail.js"></script>
<script src="/scripts/dadamail-modal.js"></script>

Where /scripts/dadamail-modal.js contains (it makes no difference whether the script is actually in the header or just linked - same result):

// Dadamail Modal Subscription Form Script

$(document).ready(function() {

                var targetDiv = '#dada_mail_modal_subscription_form_container';
                /* Initialize: */
                if($(targetDiv).length) {
                    $(targetDiv).DadaMail(
                            {
                                targetForm: 'dada_mail_modal_subscription_form'
                            }
                    )

                    /* Create the form: */
                    $(targetDiv).DadaMail('Modal');
                }
        });

And this is the subscription form:

<form action="/cgi-bin/dada/mail.cgi" method="post" accept-charset="UTF-8" id="dada_mail_modal_subscription_form"><input type="hidden" name="flavor" id="ddm_flavor" value="subscribe" ><input type="hidden" name="list" id="ddm_list" value=“SHORTNAME"><input type="email" name="email" id="ddm_email" value="" required="required" placeholder="Your email address..."><input type="submit" value="Subscribe"class="button success expand" id="ddm_subscribe_button”>

The code on the site actually has a path with the correct domain but I took it out above since these archives are public. I have checked and rechecked and all of the URLs are correct. The subscription form lives on a different domain than the Dada installation. Does anything jump out at anyone that would cause the modal box not to work? The form itself is working fine, but it takes me to the dada install instead of popping up the modal box.

I’ve got minification turned off for .js files, and I do use a CDN but I’ve purged the cache there as well. The only thing I am seeing is that another instance of colorbox-min.js is being called here:
Would that be interfering? I can’t figure out why it’s there.

Warm regards,

Mary Ann
