Moshe tested it for me and found that it is a browser bug in Firefox. It’s not downloading the protected file itself, but it downloads the code for the login page in a file with the name of the file being protected (I didn’t try to open the file so it never occurred to me that when the name was correct that it wasn’t actually the protected-file). It also explains why I was getting people who said they couldn’t open the file occasionally. Chrome and Edge work as expected. 

Moshe suggested serving the PDF files with the 'Content-Disposition: attachment’ header and telling people not to right-click. 

(And this was a different install running 7.x, but given the explanation it might be the case in a new install too.)

The protection works with one exception. If there is a direct link on a non-protected page to a file download, then when I use right-click and “Save link as”, the login comes up. When I hit cancel, it lets me download the file anyway. I tested it for multiple files.
